All students, new and returning, must have the forms below on file. Please print, fill out, and sign the forms in the links below. The CAD Policies and Procedures can also be found through a link on this page. Please read it thoroughly before attending your first class. Students without a registration and release form signed by a parent will not be allowed to participate in class. Thank you for your understanding in this matter.

*Age is used as a guideline for class placement. A student may register for a different age group pending the director’s approval. STUDENTS WISHING TO REGISTER FOR THE LEVELED BALLET PROGRAM MAY BE REQUIRED TO ATTEND AN AUDITION.

* CAD reserves the right at all times to cancel or change classes, days, and times or to provide proper substitute instructors for any amount of time (including apprentice teachers).
Please note:All classes are subject to change, teachers are subject to change, birthday party and CAD Company information can be obtained in the office. A students age is used strictly as a guideline, students are placed more accordingly to ability and experience than by age. Sometimes a teacher will request that a student be placed in a different level of class after a student has started classes, this is to ensure that every student receives the training that is best for their needs. For purposes of better instruction, there is a classroom max of 8-15 students in all recreational classes. All other classes will vary in class size. If you are interested in a class that is full, inquire in the office about a waiting list. Often a student needs to change classes after the season begins, we maintain waiting lists for these purposes.
